What is this campaign about?

The Community Care Physician Network proudly presents the Let's Talk Colorectal Cancer: Championing Early Detection campaign. This initiative is designed to provide crucial support for healthcare professionals committed to advancing colorectal cancer detection, utilizing take-home colorectal screening kits, and closing care gaps.

Colorectal cancer poses a significant challenge, particularly in North Carolina, where its impact is deeply felt. Over 1.5 million individuals aged 45 and above in North Carolina are not up to date on their colorectal screenings. This figure underscores the urgent need to prioritize early detection and prevention efforts.

Why are we doing this?

Screening your patients for colorectal cancer can be a challenge. Community Care Physician Network (CCPN) is here to assist you in meeting your quality metrics for Medicare Advantage patients. We have access to Cologuard test kits from Exact Sciences, which can be sent directly to eligible patients at no cost to your practice or the patient.

Take advantage of this opportunity to receive free Cologuard Test Kits by completing this form and mailing the completed form to kreyes@communitycarenc.org. Upon completion, patients will receive a Cologuard test kit along with a letter explaining the importance of completing the test.

Achieving higher performance in colorectal screening not only brings substantial quality bonus incentives to your practice but also enhances the care you provide to your patients. Join us in the battle against colorectal cancer, championing early detection to empower our patients for healthier, happier lives.
