What is this campaign about?

The Community Care Physician Network introduces the Let's Talk Blood Pressure Control: Supporting Patients in Their Zone campaign. This initiative supports healthcare professionals working to manage patients' high blood pressure more effectively.

As per CDC statistics for 2021 in North Carolina, high blood pressure was the primary cause of 1,300 deaths and contributed to more than 25,000 fatalities from heart disease and stroke. Approximately 35% of North Carolina adults, totaling about 2.9 million individuals, have received a high blood pressure diagnosis from a healthcare professional.

 

Why are we doing this?

Evidence-based management with appropriate screening and treatment of high blood pressure can prevent strokes and heart attacks and ultimately saves lives. Healthcare professionals play a vital role in supporting patients in their journey to control high blood pressure. This involves empowering individuals through education and open communication about the risks associated with elevated blood pressure and the positive impact lifestyle changes can have. Collaboratively developing personalized care plans that consider each patient's unique health circumstances, preferences, and lifestyle are crucial for sustained effectiveness. Encouraging regular blood pressure monitoring, both in clinical settings and through tools for home use, fosters a sense of ownership over one's health. Additionally, providing guidance on lifestyle modifications–healthy eating, regular exercise, stress management, and sufficient sleep – is essential for overall blood pressure control. For those on medication, emphasizing the importance of adherence and addressing any concerns promptly ensures ongoing compliance and contributes to comprehensive patient care.
