What is this campaign about?

In recognition of National Diabetes Month, Community Care Physician Network is launching the "Let's Talk Diabetes: Living Well Beyond Diagnosis" campaign. This initiative is aimed at engaging and supporting independent primary care practices in their mission to provide better care for patients living with diabetes. According to the American Diabetes Association, diabetes is the seventh leading cause of death in the United States, with over 83,564 deaths annually. Optimal HbA1c control is a significant factor in reducing this mortality rate.

 

HbA1c Control Matters

Proper glycemic control is not just a medical term; it's a life-changing endeavor for those managing diabetes. Research shows that maintaining optimal HbA1c levels in patients with diabetes is paramount. It's not merely about managing blood sugar; it's about securing a brighter, healthier future for your patients affected by this condition.

The volume of diabetic patients under your care with controlled HbA1c levels directly impacts the quality performance metrics related to our CCPN payer contracts. Achieving higher performance in HbA1c control can lead to substantial quality bonus incentives, thus benefiting your practice financially and improving the care you provide to your patients.
